The LivingAfterWLS Personal Self-Assessment Worksheet is an exclusive tool that is successfully being used by hundreds of our community members to chart our course with WLS. The worksheet challenges us to remember our original goal, identify areas for improvement and formulate a path forward in a positive direction.

This worksheet should be used as a private tool with the intent to pursue better living with WLS. It is a solemn personal contract; a contract of honor and self-respect. Anyone brave enough to have WLS deserves to treat him or herself well and engage in appropriate long-term behaviors to make the most of life after WLS.
